This includes:

 

  • New Student Registrations
  • Change of Address
  • Private School Registration (for students in graded K-12)
  • Port Washington resident student moving from a Private School to one of the seven Port Washington Schools

  

Please understand that in accordance with Board of Education policies 5140, 5150, and 5153, no child will be admitted to school without all the necessary forms completed.

 

Several documents in the online registration portal must be downloaded and notarized prior to completing the registration process, you will be prompted to download them toward the end of the online enrollment application. (The document will pre-fill many of the required fields based on your answers to questions in the application)

 

 

Daly PreK

Children must be four years old by December 1st in the year they will begin PreK

Kindergarten Requirements

 

Children must be five years old by December 1st in the year they will begin Kindergarten. 

Immunizations

All immunizations must specify the date the immunization was administered. They also must be submitted at time of registration. Your child will not be permitted to attend school without the necessary verification of immunizations.

 
New York State requires every student entering or attending public, private or parochial school in New York State (NYS) to be immune to:
  • diptheria   
  • tetanus
  • pertussis
  • measles
  • rubella
  • poliomyelitis
  • hepatitis B
  • varicella
  • meningococcal
In accordance with Advisory Committee on Immunization Practices (ACIP) recommendations.
 
Proof that your child has received the first dose of vaccinations in each series of the required immunizations must be provided to the District within 14 days after the first day of instruction in September. Students that are new to the country or state must provide proof within thirty days.  Failure to provide proof will result in exclusion from school. 

Residency

The Port Washington Union Free School District affirms that its primary responsibility is to provide the best possible educational opportunities for those children who are legal district residents. 

RESIDENCY: According to the State Education Department residency means domicile, which requires one’s physical presence and the intention to remain there permanently. Generally, a student’s legal school district residence is presumed to be that of their parent(s)/ legal guardian.  

 

PROOF OF RESIDENCY: Students will not be registered to attend PWUFSD until acceptable proof of residency is provided to the district.  In the registration application parent/guardians will confirm their residency and submit the notarized statement called Residency Verification Form A, page 2 and either Form B or C depending upon their type of residence.


GUARDIANSHIP: When an application is made for a student in the PWUFSD by a resident of the district who is NOT the student’s natural or adopted parent or legal guardian, the non-judicial custodial guardian must have the full responsibility for the care and custody of the child to the exclusion of all other persons for the student to be considered a resident of the district. This must be documented by legal court papers if applicable. Parents may not assign guardianship for the purpose of a student(s) to attend school in the district.

Middle School

 

Students planning to enter Weber Middle School (grades six through eight) in September can register beginning in March. During the registration process you will be asked to complete a Course Request for a Foreign Language and a Music program choice. In both instances there are dropdowns that will list the specific programs offered.

 

Early registration is encouraged to ensure that middle school students can attend orientation sessions and have their schedules processed along with continuing students. Students transferring from another middle school during the school year must register as soon as possible and are asked to bring their school records with them.

 


ALL MIDDLE SCHOOL STUDENTS MUST UPLOAD A COPY OF THEIR CURRENT TRANSCRIPT OR REPORT CARD IN ADDITION TO ALL OTHER REGISTRATION DOCUMENTS.

 

High School


Registration applications for students entering Schreiber High School in September will be accepted in the beginning of March.


ALL HIGH SCHOOL STUDENTS MUST UPLOAD A COPY OF THEIR CURRENT TRANSCRIPT OR REPORT CARD IN ADDITION TO ALL OTHER REGISTRATION DOCUMENTS.

 

- Please make sure  you have access to your email account to complete the registration process.

- Make sure your pop-up blockers are turned off

- It is NOT recommended to do this on your phone as it may be too small to read. 

New to the district

The Office of Central Registration is in the Administrative Annex at 90 Avenue C. There is a separate entrance located on the south side of Daly Elementary School.
